Study on the correlation between psychological quality,self-regulation fatigue and sleep quality of medical staff in emergency department

Abstract: Objective To investigate the current situation and correlation of psychological quality,self-regulation fatigue and sleep quality of medical staff in emergency department of tertiary public hospitals,and to explore the influence of psychological quality and self-regulation fatigue on sleep quality. Methods Convenience sampling method was used to select medical staff in emergency departments of 6 tertiary public hospitals in Mianyang City as the survey objects from June to July 2023. Psychological quality scale,self-regulated fatigue scale and sleep quality scale were used to investigate 247 medical staff in emergency department. Multiple linear regression analysis was used to explore the influencing factors of sleep quality of medical staff in emergency department. Results The Pittsburgh sleep quality index scale(PSQI) score of 247 emergency medical staff was (8.19±4.16)points, of which 158(63.97%) emergency medical staff had a total PSQI score of ≥5 points and had sleep disorders. The score of psychological quality was (285.49±55.92)points. The score of self-regulation fatigue was (45.42±15.54)points. Univariate analysis showed that ages,educational level,job categories,professional titles,length of service in the emergency department,frequency of night shifts(weeks),average overtime hours,health status,frequency of exercise(weeks/times),alcohol consumption,and coffee consumption affected the sleep quality of medical staff in the emergency department(all P<0.05). Correlation analysis showed that the total score of PSQI and the scores of each dimension were negatively correlated with the total score of psychological quality and the scores of each dimension(r=-0.206--0.713,all P<0.05),ans positively correlated with the total score of self-regulation fatigue and its dimension(r=0.241-0.605,all P<0.05). Multivariate analysis showed that education level,length of service in emergency department,night shift frequency,health status,alcohol and coffee consumption,psychological quality and self-regulation fatigue were the main influencing factors on the sleep quality of medical staff in emergency department(all P<0.05),which explained 65.7% of the total variables. Conclusion The sleep quality of medical staff in the emergency department of tertiary public hospitals is poor,among which psychological quality and self-regulation fatigue are important protective factors. Hospital managers should pay attention to the sleep health status of medical staff in special departments,improve their psychological quality and self-regulation fatigue ability,so as to improve their sleep quality.
