Mediating role of post-traumatic stress disorder on rumination thinking and job burnout in surgical nurses

Abstract: Objective To explore the correlation between rumination thinking and job burnout in surgical nurses,analyze the mediating role of post-traumatic stress disorder between the two. Methods Using a multi-stage sampling method,the general information questionnaire,rumination response scale(RRS),post traumatic stress disorder checklist civilian version(PCL-C),and Maslach burnout inventory general survey(MBI-GS) were applied to investigate 318 surgical nurses in four grade-A tertiary hospitals in western Hubei,northern Hubei,central Hubei and eastern Hubei from September to December 2023.The correlation of rumination,post-traumatic stress disorder and job burnout among nurses was analyzed by Pearson correlation.AMOS 23.0 was used to establish a structural equation model to examine the mediating effect of post-traumatic stress disorder on the relationship between rumination and burnout in surgical nurses. Results A total score of 318 surgical nurses was(45.50±14.95)points on the RRS scale,(39.89±12.98)points on the PCL-C scale,and(75.26±20.91)points on the MBI-GS scale. The ruminative thinking and post-traumatic stress disorder were positively correlated with job burnout in surgical nurses(r=0.72,0.80,both P<0.01). The post-traumatic stress disorder played a complete mediating role in the relationship between ruminative thinking and job burnout in surgical nurses(β=1.017,P<0.01),the mediating effect accounted for 95.9% of the total effect,and the mediating effect Bootstrap 95%CI was 0.885-1.143. Conclusion The nursing managers should pay attention to the risk groups of surgical nurses' ruminative thinking,identify and regularly evaluate their level of ruminative thinking,focus on the counseling and psychological support of the risk groups,formulate personalized interventions,reduce the impact of post-traumatic stress disorder on surgical nurses,and take corresponding measures to alleviate the ruminative thinking of nurses,alleviate the job burnout of surgical nurses,and stabilize the nursing team.
