Abstract: Objective To construct a survey questionnaire on the usage intention for digital health services. Methods A multi-method approach was employed, including a literature search in CNKI and Web of Science core collection databases, followed by CiteSpace analysis, literature review, and group discussion to draft the questionnaire. Two rounds of expert correspondence inquiries were conducted using the Delphi method. After revision,pre-surveys and reliability and validity tests were carried out. Results The response rate for both expert correspondence inquiries was 100%,with an authority coefficient of 0.91. Kendall’s W coefficients were 0.344 and 0.289 for the dimensions of the two rounds of inquiry questionnaire, and 0.386 and 0.486 for the items. The final questionnaire included 5 dimensions and 20 items. The content validity indices ranged from 0.86 to 1.00 for each dimensions, with items content validity indices all of 1.00,and Cronbach’s α at 0.88. Conclusion A digital health service usage intention questionnaire with good scientificity and reliability has been constructed, which can provide an effective evaluation tool for future research related to digital health.
