Investigation on homocysteine and blood lipid levels among health examination population in Xi'an City

Abstract: Objective To understand the current status and risk factors of homocysteine(HCY) and dyslipidemia among health examination population in Xi'an City,provide evidence for comprehensive prevention and control measures for healthy people. Methods A total of 3 596 people who underwent health examination in the Examination Center of the First Affiliated Hospital of Xi'an Jiaotong University from June 2020 to May 2024 were collected and divided into three age groups according to age:18- <45,45-<60 and ≥60 years old. The distribution of HCY and six items of blood lipid in different sex and age groups was analyzed. The logistic regression analysis was used to screen out the risk factors affecting the elevation of HCY,and Spearman correlation analysis was conducted to determine the correlation between apolipoprotein A and HCY level. Results The levels of age, triglyceride(TG), HCY, apolipoprotein B(APOB), non-high lipoprotein(NHDL),remnant cholesterol(RC) and arteriosclerosis index(AI) in male population[50(41, 59)years old, 1.45(1.01,2.11)mmol/L, 15.10(12.50,19.20)μmol/L,0.89(0.74, 1.03)g/L,3.42(2.87,4.02)mmol/L, 0.51(0.39,0.66)mmol/L and 3.00(2.32,3.68)] were higher than those in female population[47(37,58)years old, 1.04(0.72,1.51)mmol/L, 13.90(11.20,17.70)umol/L,0.81(0.68,0.98)g/L,3.29(2.73,3.94)mmol/L,0.44(0.34,0.56)mmol/L and 2.29(1.77,2.93)](all P<0.01). The levels of high density lipoprotein(HDL),apolipoprotein A(APOA) and total cholesterol(TC) in male population[1.15(1.01,1.33)mmol/L,1.25(1.15,1.36)g/L and 4.60(4.03,5.22)mmol/L] were lower than those in female population[1.44(1.25,1.66)mmol/L,1.41(1.29,1.52)g/L and 4.75(4.17,5.42)mmol/L](all P<0.01). There were statistically significant differences in LDL,TG,APOA,APOB,TC,NHDL and AI between males and females of different age groups(all P<0.01). In 18-<45,45-<60 and ≥60 years old groups,the level of HCY in males was higher than that in females,and the level of APOA was lower than that in females. There were statistically significant difference in all indexes between males and females in 18-<45 years old group. In 45-<60 years old age group,except APOB,the differences in all indexes were statistically significant between male and female groups. In ≥60 years old age group,the differences in all indexes were statistically significant between male and female groups(P<0.05). The multivariate analysis suggested that age and male were independent risk factors for elevated HCY level[OR(95%CI)=1.013(1.011-1.090),1.358(1.152-1.559)],and higher APOA was a protective factor for elevated HCY level[OR(95%CI)=0.499(0.268-0.941)]. The Spearman correlation analysis showed that the level of APOA was negatively correlated with the level of HCY(r=-0.043,P<0.05). Conclusion The blood lipid level and its ratio in healthy adults in Xi'an City are different in different ages and genders. The rate of dyslipidemia is high in the population,and the level of APOA is negatively correlated with the level of HCY. The low APOA is an independent risk factor for the increase of HCY. Therefore,it is necessary to carry out health lectures,free testing and other measures to enhance the public's awareness of proactive prevention and control,and conduct regular blood lipid testing in high-risk group.
