Evaluation of fairness in health resource allocation and analysis of regional differences in Tianjin City

Abstract:

Objective To explore the current situation and fairness of health resource allocation in Tianjin City,provide theoretical reference for the rational allocation of health resources in Tianjin City during the 15th Five Year Plan period. Methods Gini coefficient,Lorenz curve,and health resource agglomeration degree were used to evaluate the fairness of health resource allocation in Tianjin City from 2012 to 2022. The spatial autocorrelation analysis was used to explore the spatial distribution and changes of health resources in Tianjin City from 2012 to 2022. Results The Gini coefficient for the allocation of health resources in Tianjin City based on population was <0.4,while the Gini coefficients for the allocation based on area and GDP were both >0.4. Except for the health resource agglomeration degree(HRAD)/population agglomeration degree(PAD) in Hedong District,all six districts within the city had a HRAD/PAD value >1. The HRAD/PAD values for the number of beds,practicing(assistant) physicians,and nurses in health institutions in Hedong District,the five suburban districts,and the Binhai New Area were all <1. The global Moran index of various health resources in Tianjin City from 2012 to 2022 generally showed an upward trend year by year. Conclusions There are significant differences in the allocation of various health resources in Tianjin City,fairness of health resource allocation needs to be improved,and the trend of spatial agglomeration of health resources is intensifying. When allocating health resources,the joint impact of population,geographical area,and economy should be fully considered,and regional differences should be taken into account to share high-quality health resources.
