ICU护士共情疲劳发生现状的meta分析

摘要： 目的系统评价重症监护室（intensive care unit,ICU）护士共情疲劳的发生现状。方法检索Web of Science、PubMed、Embase、The Cochrane Library、CHINHAL、PsyclNFO、中国知网、万方、维普网及CBM数据库中ICU护士共情疲劳相关研究,时限为建库至2023年1月,使用Stata 14.0软件进行meta分析。结果最终纳入27篇文献,共计9 092名ICU护士。结果显示,ICU护士共情满意、倦怠和二次创伤得分分别为32.95（95%CI：30.44~35.46）、27.6（95%CI：26.28~28.96）、26.22（95%CI：24.75~27.70）。亚组分析结果显示,未婚、本科学历、工作年限>10年、夜班频率≤4次/月和美洲地区护士的共情满意得分较高,未婚、专科学历、工作年限6~≤10年、夜班频率>4次/月和亚洲地区护士的倦怠和二次创伤得分较高。结论 ICU护士存在中度水平的共情疲劳,未婚、专科学历、工作年限6~≤10年、夜班频率>4次/月和亚洲地区护士的共情疲劳水平更高。

Abstract: Objective To systematically evaluate the status of compassion fatigue among intensive care unit（ICU） nurses. Methods Cross-sectional studies related to the status of compassion fatigue among ICU nurses were retrieved from Web of Science,PubMed,Embase,The Cochrane Library,CHINHAL,PsyclNFO,China National Knowledge Infrastructure（CNKI）,Wanfang,VIP and China Biology Medicine disc（CBM） databases from database establishment to January 2023. The meta-analysis was performed by using Stata 14.0 software. Results A total of 27 studies involving 9 092 ICU nurses were included. The results showed that the scores of compassion satisfaction,burnout,and secondary trauma were 32.95（95%CI：30.44-35.46）,27.62（95%CI：26.28-28.96） and 26.22（95%CI：24.75-27.70）,respectively. Subgroup analysis showed that the compassion satisfaction score was higher among nurses who were unmarried,with a bachelor′s degree,nursing experience more than 10,monthly night shift ≤4 and from America region,the scores of burnout and secondary trauma were higher among nurses who were unmarried,with college degree,nursing experience of 6-≤10 years,monthly night shift more than 4 and from Asia regions. Conclusion ICU nurses have moderate levels of compassion fatigue,with higher levels of compassion fatigue among unmarried,with college degree,nursing experience of 6-≤10 years,monthly night shift more than 4 and from Asia regions.
