榆林市三级甲等医院护士工作不安全感现状及影响因素分析

Analysis on status quo and influencing factors of nurses' sense of job insecurity in tertiary hospitals in Yulin City

摘要: 目的 对榆林市三级甲等医院护士工作不安全感现状进行调查并进一步分析其影响因素,为改善护士工作不安全感现状及建设护理团队提供参考依据。方法 采用便利抽样法于2022年11月—2023年3月对榆林市三级甲等医院护士进行问卷调查,问卷内容包括一般资料和职业安全。结果 408名护士工作不安全感总得分为(129.93±26.08)分,处于中等水平;医患关系得分为(35.53±7.35)分,专业发展得分为(14.41±4.8)分,薪酬晋升得分为(16.12±4.64)分,过度竞争得分为(13.08±3.38)分,同事关系得分为(9.46±3.67)分,上下级关系得分为(8.09±2.83)分,健康风险得分为(10.79±2.94)分,制度环境得分为(12.90±3.01)分,工作丧失得分为(9.55±2.22)分。线性回归分析显示,学历是工作不安全感、医患关系和工作丧失的主要影响因素,职称是医患关系和制度环境的主要影响因素,职务是同事关系、薪酬晋升和健康风险的主要影响因素,婚姻、生育状况和工作年限是工作丧失的主要影响因素(均P<0.05)。结论 榆林市三级甲等医院护士工作不安全感处于中等水平,明显缺乏的护士占71.8%,组织管理者应增加对护士职业安全的重视,可通过合理工作安排和提供相应保障等进行改善,从而稳定护理团队。

Abstract: Objective To investigate the status quo of nurses' sense of job insecurity in tertiary hospitals in Yulin City and further analyze its influencing factors,aiming to provide a reference for improving the status quo of nurses' lack of job insecurity sense and building nursing teams. Methods A convenience sampling method was used to conduct a questionnaire survey among nurses in tertiary A hospitals in Yulin City from November 2022 to March 2023,including general information and occupational safety. Results The total score of 408 surveyed nurses' sense of job insecurity was (129.93±26.08)points,which was at a medium level. The scores of doctor-patient relationship was (35.53±7.35)points,professional development was (14.41±4.8)points,salary promotion was (16.12±4.64)points,excessive competition was (13.08±3.38)points,colleague relationship was (9.46±3.67)points,subordinate relationship was (8.09±2.83)points,health risk was (10.79±2.94)points,institutional environment was (12.90±3.01)points and job loss was (9.55±2.22)points. Linear regression analysis showed that,academic qualifications were the main influencing factors of job insecurity,doctor-patient relationship and job loss. Professional title was the main influencing factor of doctor-patient relationship and institutional environment. Position was the main influencing factor of colleague relations,salary promotion and health risks. Marital,childbearing status and working years were the main influencing factors of job loss(all P<0.05). Conclusion The job insecurity among nurses in Yulin's tertiary hospitals is at a medium level,with 71.8% of the nurses clearly lacking. Organizational managers should pay more attention to the occupational safety of nurses,and can improve it through reasonable work arrangements and providing corresponding guarantees,so as to stabilize the nursing team.
