2017年滇中滇西居民砷硒内暴露水平分析

Analysis on internal exposure to arsenic and selenium in residents of central and western Yunnan in 2017

摘要: 目的 掌握滇中、滇西常住居民砷、硒内暴露水平及其地区和人群分布特征。方法 采用多阶段复杂抽样、成比例概率抽样以及分层随机抽样,在云南省选取监测点、调查单元和调查对象。按照知情同意原则,采集调查对象外周静脉血和尿样,并分装、转运、实验室检测。利用Excel、SPSS 25.0和GraphPad Prism 8进行数据整理与分析。结果 2017年共调查云南省滇中、滇西常住居民720人,有效血样、尿样分别为709、719人份。血砷、尿砷中位数分别为2.40、19.38 μg/L,血硒、尿硒分别为82.40、14.56 μg/L。不同地区间、城乡间常住居民血砷、尿砷、血硒、尿硒浓度比较,差异均有统计学意义(均P<0.05);不同年龄段、不同性别常住居民血砷、尿砷浓度比较,差异均有统计学意义(均P<0.05);男性尿硒浓度低于女性(P<0.01)。结论 砷、硒内暴露水平在滇中、滇西常住居民中具有地区和人群分布差异,滇中常住居民血砷、血硒较滇西高。

Abstract: Objective To grasp the internal exposure to arsenic(As) and selenium(Se) in the permanent residents in central and western Yunnan,as well as regional and population distribution characteristics. Methods The multi-stage complex sampling,proportional probability sampling,and stratified random sampling were adopted to determine the monitoring points,survey units and respondents in Yunnan Province.In accordance with the principle of informed consent,the peripheral venous blood and urine samples of the subjects were collected,packed,transported,and tested of As and Se in the laboratory.Microsoft Excel,SPSS 25.0 and GraphPad Prism 8 were using for data sorting and analysis. Results A total of 720 permanent residents incentral and western Yunnan Province were surveyed in 2017,then the number of valid blood samples and urine samples were 709 and 719,respectively.The median of As in blood and urine was 2.40 and 19.38 μg/L respectively,while that of Se was 82.40 and 14.56 μg/L respectively.There were statistically significant differences in the concentrations of blood As,urine As,blood Se and urine Se among residents in different regions,and between urban and rural residents(all P<0.05). There were statistically significant differences in the concentrations of blood As and urine Se among residents in different age and gender(all P<0.05). The urine Se concentration of males was lower than that of females(P<0.01). Conclusion The regional and population distribution differences of internal exposure to As and Se are found among the permanent residents in central and western Yunnan,and the blood As and Se of the residents in central Yunnan are higher than those in western Yunnan.
