深圳市2型糖尿病及合并症患者健康相关生命质量研究

Study on health-related quality of life of patients with type 2 diabetes and its complications in Shenzhen City

摘要: 目的 分析深圳市糖尿病人群健康相关生命质量及其影响因素。方法 于2021年9—12月对深圳市罗湖区各大医院及社区卫生中心展开调研,最终确定634名18~92岁的2型糖尿病患者为研究对象。采用欧洲质量-5维度-5水平健康量表(European quality-5 dimension-5 levels questionnaire,EQ-5D-5L)、视觉检查评分(visual analogue scale,VAS)测算患者健康效用值。利用t检验及方差分析进行单因素检验,Tobit回归探究影响因素。结果 本研究人群平均EQ-5D-5L效用值为0.888±0.205,VAS得分为(76.471±17.682)分。此外,研究测算了糖尿病合并其他疾病患者的效用值,高血压为0.873±0.014,高血脂为0.886±0.012,心力衰竭为0.792±0.083,心肌梗死为0.887±0.083,中风为0.569±0.070,冠心病心绞痛为0.765±0.067,慢性胃肠道疾病为0.840±0.036,生殖器疾病为0.847±0.043,视网膜病变为0.809±0.041,肾衰竭为0.699±0.110,外周血管疾病为0.821±0.055,皮肤溃疡为0.771±0.064,癌症为0.538±0.139。单因素分析结果表明,年龄、是否住院、空腹血糖值、学历、是否饮酒、是否吸烟、是否锻炼、有无合并症、有无脑卒中、有无冠心病心绞痛、有无肾衰竭、有无皮肤溃疡、有无癌症之间健康效用值的差异均有统计学意义(均P<0.05)。回归模型结果表明,年龄大、学历低、锻炼少、住院、合并症、中风、癌症是糖尿病患者健康效用值低的影响因素(均P<0.05)。结论 深圳市2型糖尿病患者的健康相关生命质量低于正常人群,合并疾病对糖尿病患者生命质量影响大,应重视提高患者的自我管理与疾病防护意识。

Abstract: Objective To analyze the health-related quality of life and its influencing factors of diabetic population in Shenzhen City. Methods A survey was conducted on major hospitals and community health centers in Luohu District of Shenzhen City from September to December 2021,and 634 type 2 diabetes patients aged 18-92 years old were finally selected as the search objects.The European quality-5 dimension-5 levels questionnaire(EQ-5D-5L) scale,and visual analogue score(VAS) were used to measure the patient's health utility.The univariate tests were performed using t-test and analysis of variance,and Tobit regression was used to explore influencing factors. Results The mean EQ-5D-5L utility of the study population was 0.888±0.205,and the VAS score was 76.471±17.682 points.In addition,the study calculated the utility of patients with diabetes and other diseases,specifically:hypertension was 0.873±0.014,hyperlipidemia was 0.886±0.012,heart failure was 0.792±0.083,myocardial infarction was 0.887±0.083,stroke was 0.569±0.070,coronary heart disease angina was 0.765±0.067,chronic gastrointestinal disease was 0.840±0.036,genital disease was 0.847±0.043,retinopathy was 0.809±0.041,renal failure was 0.699±0.110,peripheral vascular disease was 0.821±0.055,skin ulcers was 0.771±0.064,cancer was 0.538±0.139.The results of univariate analysis showed that there were statistically significant differences in health utility among different groups in age,whether hospitalization,fasting blood glucose,education,drinking,smoking,exercise,comorbidities,stroke,coronary heart disease angina,renal failure,skin ulcer and cancer(all P<0.05). The regression model results showed that older age,lower education,less exercise,hospitalization,comorbidities,stroke,and cancer were the influencing factors of low health utility in diabetic patients(all P<0.05). Conclusion The health-related quality of life of patients with type 2 diabetes in Shenzhen City is lower than that of the normal population,and the comorbid diseaseshave a great impact on the quality of life of patients with diabetes,so it is necessary to pay attention to improving patients' self-management and disease prevention awareness.
