职业噪声对我国工人心电图影响的meta分析

Effect of occupational noise on electrocardiogram of workers in China:A meta analysis

摘要: 目的 系统评价职业噪声对我国工人心电图影响。方法 以“噪声”和“心电图”为关键词,检索CNKI、万方、VIP、CBM及Pubmed数据库2002—2021年公开发表的职业噪声对我国工人心电图影响的相关性研究,利用Stata MP 16版本软件进行meta分析。结果 本研究最终纳入文献25篇,共计17 785例,噪声暴露组10 083例,对照组7 702例。噪声暴露组与对照组心电图检出异常率的合并效应量OR值为2.34,95%CI为2.02~2.69(P<0.05),经过异质性检验后删除6篇异质性较大文献,结果显示,噪声暴露组与对照组心电图检出异常率的合并效应量OR值为2.14,95%CI为1.93~2.37(P<0.05),且暴露组窦律失常、传导异常、心脏肥大、高电压、ST-T改变发生率均高于对照组(P<0.05)。结论 噪声暴露是我国工人心电图异常的危险因素,应加强对工人噪声暴露的保护,定期进行健康检查。

Abstract: Objective To systematically evaluate the effect of occupational noise on electrocardiogram(ECG) of workers in China. Methods Using "noise" and "electrocardiogram" as keywords,the correlation studies on the effect of occupational noise on ECG of Chinese workers published in CNKI,Wanfang,VIP,CBM and Pubmed databases from 2002 to 2021 were searched. Stata MP 16 software was used for meta-analysis. Results A total of 25 of literature were included in this study,with a total of 17 785 cases, 10 083 in the noise-exposed group and 7 702 in the control group. The combined effect size(OR) of abnormal ECG detection rate in noise exposure group and control group was 2.34,95%CI was 2.02-2.69(P<0.05). After the heterogeneity test,six articles with greater heterogeneity were deleted. The results showed that the combined effect size(OR) of abnormal ECG detection rate in noise exposure group and control group was 2.14,95%CI was 1.93-2.37(P<0.05),moreover,the incidence of sinus arrhythmia,conduction abnormalities,cardiac hypertrophy,high voltage,and ST-T changes in the exposure group was higher than that in the control group(P<0.05). Conclusion Noise exposure is the risk factors of abnormal electrocardiogram in workers in China,it is necessary to strengthen the protection of workers' noise exposure and conduct regular health examination.
