我军新兵新训期间军事训练伤系统综述与meta分析

Systematic review and meta-analysis of the military training injury during the basic military training period of recruits in China

摘要: 目的 通过系统查阅我军训练伤研究相关文献,分析我军新兵新训期间训练伤发生率,为我军训练伤的防治和研究提供循证医学依据。方法 系统检索中国期刊全文数据库(CNKI)、维普、中国生物医学文献数据库(CBM)、万方和PubMed等数据库2010年1月—2023年12月期间发表的关于军事训练伤方面的中文和英文文献,筛选符合条件的文献;用美国卫生保健质量与研究机构(agency for healthcare research and quality,AHRQ)推荐的观察性研究质量评价标准对纳入的文献进行质量评价,确定纳入的研究,并摘录有关信息。通过异质性检验确定选择固定效应模型或随机效应模型;采用meta分析方法计算新兵新训期间的训练伤合并发生率。采用漏斗图法和Egger线性回归法评价发表偏倚;采用逐一剔除法进行敏感性分析。结果 本研究共纳入文献46篇,包含样本106 329例,合并计算后得到新兵新训期间训练伤发生率为20.2%(95%CI:18.0%~22.5%)。亚组分析发现,冬季新训和秋季新训训练伤发生率差异无统计学意义(P>0.05);2015年以后训练伤发生率(17.4%,95%CI:13.9%~21.0%)较2015年以前(22.2%,95%CI:19.5%~25.0%)有下降趋势,差异无统计学意义(P>0.05)。结论 我军新兵新训期间训练伤发生率为20.2%,随着近年来我军大力提升实战化训练、科学组训和后勤保障水平,训练伤有下降趋势。建议未来在常规训练基础上实施深入健康教育、有效心理干预、健康状况评估、功能性动作筛查及纠正性训练等积极干预措施,降低训练伤风险,提升训练质量。

Abstract: Objective To systematically review relevant literature on military training injuries in China,analyze the incidence of training injuries among new recruits during the basic military training period,and provide evidence-based medicine for the prevention,treatment,and research of military training injuries in China. Methods A systematically literature search on military training injuries was performed in the Chinese and English database such as CNKI,CQVIP,CBM,Wanfang,PubMed and so on,and selected eligible literature. The publication time was limited between January 2010 and December 2022. The cross-sectional study quality evaluation standard recommended by the agency for health research and quality(AHRQ) in the United States were used to evaluate the quality of the included literature,determine the included studies,and extract relevant information. Fixed effect model or random effect model was selected through heterogeneity test results. The meta-analysis method was used to calculate the combined incidence of training injuries. Funnel plot method and Egger linear regression methods were used to evaluate publication bias. The sensitivity analysis was carried out by the one-by-one elimination method. Results A total of 46 articles were included in this study,including a sample of 106 329 cases. The combined incidence of training injury during the new recruits' basic training was 20.2%(95%CI:18.0%-22.5%). Subgroup analysis showed that there was no statistically significant difference in the incidence of training injury between the recruits who were trained in winter or autumn(P>0.05). The incidence of training injury after 2015(17.4%,95%CI:13.9%-21.0%) showed a downward trend compared to before 2015(22.2%,95%CI:19.5%-25.0%),but the difference was not statistically significant(P>0.05). Conclusion The incidence of training injuries during the recruits' basic training period in China was 20.2%. With the vigorous improvement of practical training,scientific group training,and logistical support in recent years,there is a downward trend in training injuries. It is recommended to implement active intervention measures such as in-depth health education,effective psychological intervention,health status assessment,functional movement screening,and corrective training on the basis of routine training in the future,in order to reduce the risk of training injuries and improve the quality of training.
