上海市浦东新区小学生超重和肥胖与KAP相关性调查

Investigation on the correlation between overweight and obesity and KAP among primary school students in Pudong New Area of Shanghai City

摘要: 目的 了解上海市浦东新区小学生超重和肥胖与健康知识、态度、饮食行为(knowledge,attitude,Practice,KAP)的相关性,为预防小学生超重和肥胖发生、进展及开展健康教育提供参考依据。方法 采用城乡分层整群抽样方法,于2022年3月抽取上海市浦东新区城、乡各1所小学一~五年级1 236名学生进行身高和体质量测量,根据WS/T 586—2018《学龄儿童青少年超重肥胖筛查标准》筛选超重和肥胖的学生473人。经知情同意,419名超重和肥胖学生进行体成分分析、腰臀围、血压、肺活量等身体形态及体能测试指标和问卷调查。采用Spearman秩相关分析KAP与体质量指数(body mass index,BMI)的相关性。结果 2所小学超重和肥胖检出率分别为17.9%和20.4%,其中男生超重和肥胖检出率分别为20.5%和25.8%,女生超重和肥胖检出率分别为14.8%和14.1%。BMI、腰围、腰围身高比在不同年级及性别间差异均有统计学意义(均P<0.05),体脂率在不同年级间差异有统计学意义(P<0.05),在不同性别间差异无统计学意义(P>0.05)。超重和肥胖儿童BMI与体脂肪率呈正相关(r=0.702,P<0.05)。超重和肥胖儿童营养KAP总分36分,平均得分为24.75分,健康知识和KAP总分在不同年级间差异有统计学意义(P<0.05),健康态度和健康行为在不同年级和性别间差异均无统计学意义(均P>0.05),KAP与BMI存在负相关性(r=-0.098,P<0.05)。结论 KAP与小学生超重和肥胖存在一定的相关性,提高膳食营养健康教育及运动行为干预是预防小学生超重和肥胖的重要措施。

Abstract: Objective To investigate the correlation between overweight and obesity among primary school students in Pudong New Area of Shanghai City and their health knowledge,attitude,practice(KAP),in order to provide reference for preventing the occurrence and progression of overweight and obesity among primary school students and carrying out healthy education. Methods A urban and rural stratified cluster sampling method was used to select 1 236 students from grades 1 to 5 of two primary school in urban and rural areas of Pudong New Area of Shanghai in March 2022 for height and weight measurement. According to WS/T586-2018 "Screening Standards for Overweight and Obesity in School Age Children and Adolescents",473 overweight and obese students were screened. With informed consent,419 overweight and obese students underwent body composition analysis,waist and hip circumference,blood pressure,vital capacity,and other body shape and physical fitness testing indicators,as well as a questionnaire survey. Spearman rank correlation analysis was used to investigate the correlation between KAP and body mass index(BMI). Results The detection rates of overweight and obesity in the two primary schools were 17.9% and 20.4%,respectively. Among them,the detection rates of overweight and obesity in boys were 20.5% and 25.8%,respectively,and the detection rates of overweight and obesity in girls were 14.8% and 14.1%,respectively. BMI,waist circumference,and waist height ratio showed statistically significant differences among different grades and genders(all P<0.05),while body fat percentage showed statistically significant differences among different grades(P<0.05),and there was no statistically significant difference among different genders(P>0.05). There was a positive correlation between BMI and body fat percentage in overweight and obese children(r=0.702,P<0.05).The total score of nutritional KAP for overweight and obese children was 36 points,with an average score of 24.75 points. There were statistically significant differences in health knowledge and total KAP scores among different grades(both P<0.05),while there were no statistically significant differences in health attitudes and behaviors among different grades and genders(all P>0.05). There was a negative correlation between KAP and BMI(r=-0.098,P<0.05). Conclusion There is a certain correlation between KAP and overweight and obesity in primary school students. Improving dietary nutritional health education,as well as intervening in exercise behaviors,are an important measure to prevent overweight and obesity in primary school students.
