职业性慢性正己烷中毒事件回顾性分析

Retrospective analysis of occupational chronic n-hexane poisoning incidents

摘要:

目的 分析职业性慢性正己烷中毒事件的特征、原因及风险因素,为预防策略提供依据。方法 以2023—2024年发生在广东省某市慢性正己烷中毒企业和中毒患者为研究对象,收集现场职业卫生学调查、工作场所正己烷检测分析和临床资料及诊断资料,对正己烷接触导致职业性化学中毒进行原因分析。结果 3起事件均发生在使用含正己烷溶剂进行清洁和擦拭作业的小型企业,共导致7名员工中毒。职业卫生学调查显示,事件A岗位短时间接触浓度(short-term exposure concentration,CSTE)为806.1~1023.3 mg/m3,超标5.7倍;事件B岗位时间加权平均接触浓度(exposure concentration of time weighted average,CTWA)为0.8~712.0 mg/m3,超标7.1倍,CSTE为1.4~1 007.4 mg/m3,超标5.6倍;事件C岗位CTWA为34.9~89.7 mg/m3CSTE为44.2~520.7 mg/m3,超标2.9倍。原辅料分析显示,涉事企业清洁剂中正己烷峰面积百分比为10.88%~12.42%;临床诊断6例职业性慢性重度正己烷中毒,1例中度中毒,所有患者均出现典型周围神经病变,神经肌电图显示,四肢周围性神经损害。调查发现,3家企业均存在车间布局不合理、通风不良、化学溶剂使用混乱及未落实职业卫生主体责任等问题。结论 职业性慢性正己烷中毒是1种具有隐匿性和群发性的职业病,特别在小微型企业中风险尤为突出,应压实企业主体责任以防止正己烷中毒事件的发生。

Abstract:

Objective To analyze the characteristics,causes,and risk factors of occupational chronic n-hexane poisoning incidents,and provide a basis for prevention strategies. Methods A retrospective study was conducted on enterprises and patients involved in chronic n-hexane poisoning incidents that occurred in a city in Guangdong Province from 2023 to 2024. Data from on-site occupational health surveys,workplace n-hexane concentration measurements,and clinical and diagnostic records were collected to analyze the causes of occupational chemical poisoning resulting from n-hexane exposure. Results All three incidents occurred in small enterprises using n-hexane-containing solvents for cleaning and wiping operations,resulting in poisoning of 7 employees. Occupational hygiene investigations showed that incident A workstations had short-term exposure concentration(CSTE) of 806.1-1 023.3 mg/m3,exceeding the limit by a factor of 5.7;Incident B workstations had exposure concentration of time weighted average(CTWA) of 0.8-712.0 mg/m3,exceeding the limit by a factor of 7.1,and CSTE of 1.4-1 007.4 mg/m3,exceeding the limit by a factor of 5.6;Incident C workstations had CTWA of 34.9-89.7 mg/m3 and CSTE of 44.2-520.7 mg/m3,exceeding the limit by a factor of 2.9. Analysis of raw materials showed that the percentage of n-hexane peak area in the cleaning agents used by the involved enterprises ranged from 10.88% to 12.42%. Clinical diagnosis included 6 cases of severe occupational chronic n-hexane poisoning and 1 case of moderate poisoning. All patients presented with typical peripheral neuropathy,and electromyography showed peripheral nerve damage in all four limbs. The investigation identified common problems across all three enterprises,including improper workshop layout,poor ventilation,unsystematic use of chemical solvents,and a failure to fulfill their primary responsibilities for occupational health. Conclusion Occupational chronic n-hexane poisoning is an occupational disease characterized by its insidious onset and tendency for cluster outbreaks,and the risk is particularly high in small and micro-sized enterprises. The main responsibility of enterprises should be strengthened to prevent the occurrence of n-hexane poisoning incidents.
