重症监护病房护士报警疲劳的潜在剖面分析和影响因素

摘要/Abstract

摘要： 目的了解重症监护病房护士报警疲劳程度现状,为识别各亚组人口学变量分布特征而构建一个潜在的类别。方法 2022年9月—2023年2月,采用自制一般资料调查表、重症监护病房（intensive care unit,ICU）护士报警疲劳量表、中国护士工作压力源量表和认知负荷量表,以河南省5所三级甲等医院447名ICU护士为研究对象开展调查。结果重症监护病房护士的报警疲劳程度得分为（35.74±5.52）分,处于中等偏上水平,基于ICU护士报警疲劳得分识别出3个潜在类别,分别为中重度疲劳-低水平组（15.44%）、中重度疲劳-高水平组（38.7%）和中重度疲劳-中水平组（45.86%）。监护室工作年限、报警相关知识培训、报警相关不良事件经历及护士工作压力均是ICU护士报警疲劳潜在剖面分类的影响因素（均P<0.05）。结论 ICU护士报警疲劳处于中重度且分类特征明显。护理管理人员可根据监护病房报警疲劳影响因素分析构建有针对性的干预方案,降低ICU护士报警疲劳,提高监护病房患者的护理质量。

Abstract: Objective To understand the current status of alarm fatigue among nurses in intensive care units（ICU）,and to construct a potential category for identifying the distribution characteristics of demographic variables in each subgroup. Methods From September 2022 to February 2023,447 nurses from the ICU of 5 grade-A tertiary hospitals in Henan Province were cluster selected as the research objects,and the self-made general information questionnaire,ICU nurse alarm fatigue scale,Chinese nurse work stressor scale and cognitive load scale（MDT-CL） were used to investigate,and the data were analyzed. Results The alarm fatigue score of ICU nurses was （35.74±5.52）points,which was above average. Three potential categories were identified based on ICU nurse alarm fatigue scores,namely moderate to severe fatigue-low level group（15.44%）,moderate to severe fatigue-high level group（38.7%） and moderate to severe fatigue-medium level group（45.86%）. The years of work experience in ICU,training on alarm related knowledge,experience of alarm related adverse events,and nurse work pressure were all influencing factors for the potential profile classification of ICU nurse alarm fatigue（all P<0.05）. Conclusion The alarm fatigue of nurses in the ICU is moderate to severe and has obvious classification characteristics. Nursing managers can construct targeted intervention programs according to the influencing factors of alarm fatigue in the ICU,so as to reduce the alarm fatigue of nurses in the ICU and improve the quality of care for patients in the ICU.
