护士长健康生产力受损在职业压力与人文关怀品质间的中介效应

Mediating effects of health-related productivity loss in head nurses between occupational stress and humanistic care quality

摘要: 目的 分析陕西省三级甲等医院护士长健康生产力受损在职业压力与人文关怀品质间的中介效应,为提升护士长人文关怀品质提供新思路。方法 2023年11—12月采用便利抽样法选取陕西省12所三级甲等医院的248名护士长为调查对象,采用一般资料调查表、健康生产力受损量表、护士工作压力源量表、护士人文关怀品质量表进行横断面调查,并对数据进行分析。结果 护士长健康生产力受损总分为(15.53±5.95)分,人文关怀品质总分为(129.86±15.56)分,职业压力得分为(91.93±12.95)分;护士长人文关怀品质与职业压力和健康生产力受损呈负相关(r=-0.294、-0.279,均 P<0.01),职业压力与健康生产力受损呈正相关(r=0.482,P<0.01)。健康生产力受损在职业压力与人文关怀品质间起部分中介效应,占总效应的31.46%。结论 职业压力可负向预测护士长的人文关怀品质,也可通过健康生产力受损间接影响人文关怀品质,医院管理者应采取多种干预措施缓解护士长的职业压力,降低健康生产力受损的发生率,提升其人文关怀品质。

Abstract: Objective To analyze the mediating effect of health-related productivity loss between occupational stress and humanistic care quality of head nurses in grade-A tertiary hospitals in Shaanxi Province,and provide new ideas for improving the humanistic care quality of head nurses. Methods Using the convenience sampling method,248 head nurses from twelve grade-A tertiary hospitals in Shaanxi Province were surveyed by the general information questionnaire,Standford presenteeism scale,the nurses' job stressor scale and nursing caring characters assessment tool from November to December 2023,and the data were analyzed. Results The total score of health-related productivity loss among head nurses was (15.53±5.95)points,the total score of humanistic care quality was (129.86±15.56)points,and the total score of occupational stress was (91.93±12.95)points. The humanistic care quality of head nurses was negatively correlated with occupational stress and health-related productivity loss(r= -0.294,-0.279,both P<0.01),while occupational stress was positively correlated with health-related productivity loss(r=0.482,P<0.01). The health-related productivity loss partially mediated the effect between occupational stress and humanistic care quality,accounting for 31.46% of the total effect. Conclusion Occupational stress can negatively predict the humanistic care quality of head nurses,and can indirectly affect the humanistic care quality through health-related productivity loss. The hospital administrators should adopt a variety of interventions to alleviate the occupational stress of head nurses,reduce the incidence of health-related productivity loss,and enhance the quality of their humanistic care.
