Abstract: With the continuous increase in the number of elderly patients with chronic diseases, the phenomenon of social frailty of elderly patients with chronic diseases is becoming more and more significant. The social frailty is one of the important dimensions of frailty, which can lead to a decrease in the quality of life, prolongation of the disease process, and an increased risk of death in elderly patients with chronic diseases. At present, most of the studies related to social frailty in China focus on the elderly in the community, and there are relatively few studies on elderly patients with chronic diseases. Therefore, this paper reviews the assessment tools, the status quo and related factors of social frailty in elderly patients with chronic diseases, and puts forward relevant suggestions to provide reference for early identification and intervention of social frailty in elderly patients with chronic diseases.
