摘要： 目的初步探讨《精神障碍诊断与统计手册（第5版）》（diagnostic and statistical manual 5th edition,DSM-5）一级跨界症状量表成人版在学兵中的适用性,并了解其精神健康状况,为开展心理卫生工作提供依据。方法 2019年4月应用DSM-5一级跨界症状量表成人版、症状自评量表（self-reporting inventory,SCL-90）、患者健康问卷抑郁分表（patient health questionaire-9 items,PHQ-9）、广泛性焦虑障碍量表（generalized anxiety disorder,GAD-7）和一般情况调查表对某基地940名学兵精神健康状况进行评估分析。结果共回收929份有效问卷,平均年龄为（20.4±1.6）岁,男性925人,占99.6%。学兵DSM-5一级跨界症状量表成人版得分为（4.62±6.49）分,SCL-90得分为（102.10±19.80）分,PHQ-9得分为（2.60±3.14）分,GAD-7得分为（1.34±2.40）分。DSM-5一级跨界症状量表成人版中,抑郁得分为（0.49±0.66）分,愤怒得分为（0.40±0.65）分,自杀观念得分为（0.02±0.16）分。口腔溃疡和胃肠道症状者精神健康状况均差于无症状者（均P<0.01）,精神病性症状和物质使用亚量表有5个条目的统计量不理想（n'≥4）,其他18个条目得分与量表总分均呈正相关（r=0.430~0.755,均P<0.01）,项目同质性较好。量表内部一致性信度 Cronbach's α系数为 0.890,Spearman-Brown分半信度系数为0.869,与SCL-90总分效标关联系数为0.787（P<0.01）,其亚量表（除精神病性症状、物质使用）与SCL-90各亚量表相关系数最高值组合r=0.444~0.668,（均P<0.01）。结论除精神病性症状和物质使用维度外,DSM-5一级跨界症状量表成人版在学兵中的应用效果较好,量表信度及效标关联效度较好。被调查对象精神健康状况整体良好,但其抑郁和愤怒问题仍需关注。可结合学兵精神健康状况的分布特点有针对性地开展心理卫生工作。

Abstract: Objective To preliminarily explore the applicability of the diagnostic and statistical manual 5th edition（DSM-5） level 1 cross-cutting symptom measure-adult in military cadets,understand their mental health,so as to provide basis for developing the work on psychological hygiene. Methods In April 2019,the DSM-5 level 1 cross-cutting symptom measure-adult,self-reporting inventory（SCL-90）,patient health questionaire-9 items（PHQ-9）,generalized anxiety disorder（GAD-7） and general situation questionnaire were used to evaluate and analyze the mental health status of 940 military cadets in a base. Results Totally 929 valid questionnaires were collected. The average age was （20.4±1.6）years old and 925（99.6%） were male. The score of DSM-5 level 1 cross-cutting symptom measure-adult was （4.62±6.49）points,the score of SCL-90 was （102.10±19.80）points,the score of PHQ-9 was （2.60±3.14）points,and the score of GAD-7 was （1.34±2.40）points. The depression score was （0.49±0.66） points,anger score was （0.40±0.65）points,and suicidal ideation score was （0.02±0.16）points on the DSM-5 level 1 cross-cutting symptom measure-adult.The mental health of cadets with oral ulcer and gastrointestinal symptoms were worse than that of asymptomatic cadets（all P<0.01）. The statistics of five items in the sub-scale of psychotic symptoms and substance use of the DSM-5 level 1 cross-cutting symptom measure-adultwere not ideal（n'≥4）,and the correlations between the scores of the other 18 items and the total score of the scale were all significant（r=0.430-0.755,all P<0.01）,showing good item homogeneity. The scale internal consistency reliability was estimated with a Cronbach's α coefficient of 0.890,Spearman-Brown split reliability coefficient was 0.869,and the correlation coefficient with the SCL-90 total score validity was 0.787（P<0.01）. The correlation coefficients（high value） between DSM-5 sub-scales（except psychotic symptoms and substance use） and SCL-90 sub-scales were r=0.444-0.668（all P<0.01）. Conclusions The applicability of DSM-5 level 1 cross-cutting symptom measure-adultin military cadets is feasible except the dimensions of psychotic symptoms and substance use. The reliability and criterion related validity of the scale are good. The mental health of the cadets is generally good,but the problems of depression and anger still deserved special attention. The psychological service should be carried out according to the distribution characteristics of mental health of cadets.
