我国急诊科医护人员职业倦怠现状的meta分析

Meta-analysis of job burnout among emergency medical staff in China

摘要: 目的 使用meta分析方法对我国急诊科医护人员职业倦怠现状进行分析,了解其职业倦怠水平。方法 检索中国知网、维普期刊数据库、万方数据知识服务平台、Pubmed、Web of Science、the Cochrane Library、EMBASE等数据库,筛选建库至2022年2月刊发的涉及我国急诊科医护人员职业倦怠相关研究,应用Stata 16.0进行meta分析。结果 最终纳入分析文献13篇,累计研究对象3 100人。Meta分析结果显示,急诊科医护人员三维度表现分别处于中度情感衰竭(25.79,95%CI=24.03~27.56)、低度非人性化(5.01,95%CI=3.77~6.26)和重度低个人成就感(29.63,95%CI=26.51~32.74)。亚组分析显示,样本量的差异均有统计学意义(F=5.12、1.81、3.25,均P<0.05);敏感性分析表明,meta分析结果较稳定;Egger线性回归分析结果提示,低个人成就感分量表可能存在发表偏倚(t=0.98,P<0.05)。结论 2011—2022年我国急诊科医护人员职业倦怠程度较高,应深入研究影响急诊科医护人员职业倦怠的因素,采取必要的干预措施,以降低急诊科医护人员职业倦怠率,减少其危害。

Abstract: Objective A meta-analysis of job burnout among Chinese emergency physicians was conducted in order to provide reference for reasonable intervention. Methods We searched databases including CNKI,VIP,Wanfang,Pubmed,Web of Science,the Cochrane Library and EMBASE from inception through February,2022 to retrieve published papers that had estimated the burnout levels among emergency physicians. Data analysis was performed with the use of Stata16.0. Results We identified a total of 13 eligible studies involving 3100 emergency physicians. According to the meta-analysis,the mean estimate was moderate for emotional exhaustion(25.79,95%CI=24.03-27.56),mild for depersonalization(5.01,95%CI=3.77-6.26),and severe for personal accomplishment(29.63,95%CI=26.51-32.74) among emergency physicians. Subgroup analyzes showed that there was a significant difference in sample size(F=5.12、1.81、3.25,all P<0.05). Sensitivity analysis suggested that the results were stable while Egger tests demonstrated publication bias in personal accomplishment dimension(t=-0.98,P<0.05). Conclusions From 2011 to 2022,the detection level of occupational burnout among emergency physicians in China is significantly high. Factors affecting burnout among emergency physicians should be thoroughly investigated. It is critical to take measures to limit the prevalence rate and harm of burnout among emergency physicians.
