露天煤矿工人职业性肌肉骨骼疾患发生模式及影响因素分析

Analysis on incidence patterns and influencing factors of work-related musculoskeletal disorders in open-pit coal miners

摘要: 目的 了解新疆露天煤矿工人职业性肌肉骨骼疾患(work-related musculoskeletal disorders, WRMSDs)的发生情况,通过潜在类别分析(latent class analysis,LCA)识别该群体WRMSDs的分类模式,并探讨相关影响因素与WRMSDs发生模式之间的关系。方法 2023年1—10月采用整群抽样法对新疆5家大型露天煤矿企业3 590名在职员工进行问卷调查,并对数据进行分析。结果 新疆露天煤矿工人WRMSDs年症状发生人数为1 469人,发生率为40.9%,发生率最高的3个部位依次为腰部(940人,26.2%)、颈部(916人,25.5%)、肩部(782人,21.8%),髋臀部(249人,6.9%)发生率最低。新疆露天煤矿工人WRMSDs发生模式最优模型为5分类,分别为肩颈腰疼痛组(418人,11.64%)、仅腰部疼痛组(164人,4.57%)、各部位均疼痛组(157人,4.37%)、上半身疼痛组(284人,7.91%)及轻微疼痛组(2 567人,71.5%)。职业因素进入模型后,发现“有充足的休息时间”在各类别中条件概率均偏小(均ρ<0.500),“在寒冷的环境中工作”的情况在各类别中普遍存在(ρ>0.700),9种职业危险因素在各部位均疼痛组和上半身疼痛组的条件概率均较高(均ρ>0.500)。通过蓄积性疲劳分组后,除无蓄积性疲劳组分为4个类别,轻度、中度、重度蓄积性疲劳组均分为3个类别。4个组中均存在上半身疼痛组,其类别概率分别为4.28%、30.21%、37.10%、36.74%。将睡眠质量作为协变量纳入分析后,除轻度蓄积性疲劳的“各部位均疼痛组”参照“上半身疼痛组”无明显差异外,其他分类差异均有统计学意义(均P<0.01)。结论 露天煤矿工人WRMSDs发生率较高,多好发于腰、颈、肩部。暴露在不同的职业因素中会导致工人落入不同的疼痛分类。随着蓄积性疲劳水平的增加,露天煤矿工人发生上半身疼痛的概率更大。睡眠质量与WRMSDs发生模式分类有关,良好的睡眠质量可降低工人出现多部位疼痛模式的风险。

Abstract: Objective To understand the incidence of work-related musculoskeletal disorders(WRMSDs) among open-pit coal miners in Xinjiang,identify the incidence pattern of WRMSDs in this group through latent class analysis(LCA),and explore the relationship between related influencing factors and the incidence pattern of WRMSDs. Methods From January to October 2023,a cluster sampling method was used to conduct a questionnaire survey of 3 590 employees in 5 large open-pit coal mining enterprises in Xinjiang,and the data were analyzed. Results The annual symptom incidence of WRMSDs among open-pit coal miners in Xinjiang was 1 469,with an incidence rate of 40.9%. The top three body parts with the highest incidence rates were the waist (940,26.2%),the neck(916,25.5%) and the shoulder(782,21.8%),the hip and buttocks(249,6.9%) with the lowest incidence rate.The optimal model for the incidence patterns of WRMSDs among open-pit coal miners in Xinjiang was a five-category model,which included the following groups respectively:the shoulder-neck-waist pain group(418,11.64%),only waist pain group (164,4.57%),the pain in all body parts group(157,4.37%),the upper body pain group(284,7.91%),and the mild pain group(2 567,71.5%). After occupational factors were incorporated into the model,it was found that the conditional probability of "having sufficient rest time" was relatively low in all categories(all ρ<0.500),and the situation of "working in a cold environment" was commonly seen in all categories(ρ>0.700). The conditional probabilities of the 9 occupational risk factors were relatively high in both the pain in all body parts group and the upper body pain group(all ρ>0.500).After grouping by cumulative fatigue,except that the non-cumulative fatigue group was divided into four categories,the mild,moderate and severe cumulative fatigue groups were each divided into three categories. The upper body pain group existed in all four groups,and its category probabilities were 4.28%,30.21%,37.10% and 36.74%,respectively.After incorporating sleep quality as a covariate into the analysis,except that there was no significant difference between the "pain in all body parts group" and the "upper body pain group" in the mild cumulative fatigue group,all other classification differences were statistically significant(all P<0.01). Conclusion The incidence rate of WRMSDs among open-pit coal miners is relatively high,and it often occurs in the waist,neck and shoulder areas. Exposure to different occupational factors can lead workers to fall into different pain classifications. As the level of cumulative fatigue increases,the probability of open-pit coal miners experiencing upper body pain becomes greater. Sleep quality is related to the classification of the incidence patterns of WRMSDs,and good sleep quality may reduce the risk of workers having the pain pattern in multiple body parts.
