Objective To analyze the prevalence and influencing factors of localized work fatigue after continuous work among laborers in the metal products industry in Qingpu District of Shanghai City. Methods Data were collected from the work-related diseases surveillance questionnaire of key industries in Qingpu District of Shanghai City, and a total of 522 valid questionnaires were collected, including 359 males(68.77%) and 163 females(31.23%). Binary Logistic regression was conducted to explore the influencing factors. Results The incidence of localized work fatigue after continuous work among laborers was 51.7%. Logistic regression analysis revealed that workers with higher educational level and longer cumulative rest duration had a lower risk of localized work fatigue. For each unit increase, the risk of localized fatigue decreased by 0.628 times(95%CI:0.465-0.850) and 0.694 times(95%CI:0.504-0.957),respectively.Workers with poor physical condition, history of trauma,or musculoskeletal pain had a significantly higher risk of localized fatigue, with odds ratios of 2.650(95%CI:1.709-4.108),3.321(95%CI:1.087-10.146),and 5.834(95%CI:3.225-10.553),respectively,compared to those without these conditions. Laborers who had a higher frequency of physical exercise after work and more frequent upper limb or hand force demand at work were at a higher risk of localized work fatigue. For each unit increase, the risk of localized fatigue increased by 1.403 times(95%CI:1.015-1.940) and 1.361 times(95%CI:1.080-1.716),respectively. Working in an uncomfortable posture, sufficient rest time, and continued working after rest were factors that increased risks of localized work fatigue, with odds ratios of 4.327(95%CI:1.817-10.302),0.461(95%CI:0.283-0.750),and 3.358(95%CI:1.586-7.112),respectively,compared to workers who did not have these conditions. Besides, laborers engaged in three-shift work had a higher risk of localized work fatigue[4.211 times(95%CI:1.866-9.503)] compared to those who did not work in shifts. Conclusion Enterprise managers can alleviate the occurrence of localized fatigue of workers, thus improve production efficiency and quality by enhancing the working environment, optimizing work processes, and providing appropriate rest time.

Objective To analyze lung function and hematological parameter changes in patients with silicosis at different stages, and their relationship with interleukin-1β(IL-1β) and interleukin-18(IL-18) levels, and to understand the application value of a biomarker evaluation system based on principal component analysis in the staging assessment of silicosis. Methods A cross-sectional study was conducted involving occupational dust-exposed individuals, including coal mine administrative staff, undiagnosed dust-exposed subjects, and patients with stage I-III silicosis, who underwent occupational health examinations and treatment at a hospital in Xinjiang from January 2020 to December 2024. Forced vital capacity(FVC),forced expiratory volume in one second(FEV1),and routine hematological parameters were measured. Serum IL-1β and IL-18 levels were determined using enzyme-linked immunosorbent assay(ELISA).The Jonckheere-Terpstra trend test was applied to analyze parameter trends, while Spearman correlation analysis was used to assess associations between inflammatory cytokines and pulmonary function indices. Principal component analysis(PCA) was employed to construct a multidimensional biomarker evaluation model. Results As the disease progressed, pulmonary function indicators of patients showed a progressive decline, with FVC decreasing by 41.6%(P<0.01);Serum inflammatory cytokines increased markedly, with IL-1β rising from 0.15 pg/mL in control group to 7.28 pg/mL in stage III silicosis(a 48.5-fold increase) and IL-18 increasing from 136.89 pg/mL to 1187.39 pg/mL(an 8.7-fold increase),with JT values of 8929 and 6744.5,respectively(both P<0.01);In hematological parameters, while lymphocyte percentage declined from 32.98% to 24.4%(P<0.05).Correlation analysis revealed that IL-1β was significantly negatively correlated with FVC(ρ=-0.192,P<0.01). PCA extracted three principal components with a cumulative variance contribution of 97.38%,where principal component 1(PC1) primarily reflected the intensity of inflammatory responses, and principal component 2(PC2) represented the functional status of the hematopoietic system. Conclusion Pulmonary function in patients with silicosis exhibits stage-specific decline, while hematological findings indicate an enhanced inflammatory response. Serum IL-1β and IL-18 levels are closely associated with disease severity and show significant negative correlations with pulmonary function impairment. The multidimensional biomarker evaluation system based on PCA integrates inflammatory markers, hematological parameters, and pulmonary function indices, offering a novel tool for staging assessment and individualized monitoring of silicosis.

Objective To understand the differences of common serum biomarkers between patients with silicosis and healthy populations, analyze the relationship between relevant indicators and the occurrence and development of silicosis, and evaluate their clinical value as potential diagnostic biomarkers for silicosis. Methods Clinical data were collected from hospitalized male patients clinically diagnosed with silicosis at Dazhou Central Hospital from January 2015 to December 2022. A total of 499 male silicosis patients with complete medical records were selected as the observation group, while age-matched healthy males undergoing physical examinations served as the control group, and the data were analyzed. Results The MP25P75) of direct bilirubin[3.3(2.4,4.6)umol/L],creatinine[75.6(66.0,87.0)umol/L],urea[6.2(4.9,7.9)umol/L],neutrophil-to-lymphocyte ratio(NLR)[3.4(2.4,5.3)],and platelet-to-lymphocyte ratio(PLR)[126.3(86.2,182.6)] in the observation group were higher than those in the control group, while hemoglobin[134.0(122.0,145.0)g/L] was lower than in the control group, and the differences were all statistically significant(all P<0.05). Except for the urea index, the differences in other indicators were statistically significant (all P<0.05),indicating that they were meaningful predictors of silicosis. Bivariate correlation analysis indicated that hemoglobin was negatively correlated with the prediction of silicosis(r=-0.292,P<0.05),while direct bilirubin, creatinine,PLR,and NLR(r=0.139,0.113,0.281,0.264,all P<0.05) were all positively correlated with the prediction of silicosis. The receiver operating characteristic(ROC) curve showed that the area under the curve(AUC) for NLR was 0.733,with specificity of 0.740 and sensitivity of 0.630.The AUC for PLR was 0.731,with specificity of 0.591 and sensitivity of 0.775. The AUC for direct bilirubin was 3.690,with specificity of 0.812 and sensitivity of 0.402. The AUC for creatinine was 0.592,with sensitivity of 0.334 and specificity of 0.818;The AUC for hemoglobin was 0.288,with specificity of 0.994 and sensitivity of 0.012. The AUC for the combined detection of the above significant indicators was 0.862,with specificity of 0.929 and sensitivity of 0.667. The combined detection of these indicators showed significantly higher predictive efficacy for silicosis than any single test alone. Conclusion Elevated levels of bilirubin, urea, creatinine, and blood cell counts in the serum of silicosis patients may serve as potential biomarkers for diagnosing silicosis. The combined detection of bilirubin, creatinine, and blood cell indices can improve the predictive efficacy for silicosis.

Methods Data on the age-standardized prevalence and DALY rates for hearing loss attributed to occupational noise among people aged 20 to 64 were obtained from the Global Burden of Disease(GBD) 2021 database for both global and Chinese populations. The estimated annual percentage change(EAPC) and its 95% confidence interval(CI) were calculated to assess trends in the disease burden. The Bayesian age-period-cohort(BAPC) model was used to analyze changes in age-standardized DALYs and to forecast disease trends from 2020 to 2030. Results From 2007 to 2021,the age-standardized prevalence of hearing loss attributed to occupational noise in China increased from 24 471.01 per 100 000 population to 25 252.67 per 100 000 population(EAPC= 0.258%,P<0.05). The age-standardized DALY rate also rose from 561.24 per 100,000 to 582.15 per 100,000(EAPC=0.326%,P<0.05),with a growth rate higher than the global average. The BAPC model indicated that the risk of occupational noise-induced hearing loss in China increased with age from 2007 to 2021,with females experiencing a higher risk than males. Notably, the DALYs rates were significantly higher among those aged 20-<25 and 30-<40 years. Although the period-cohort trend showed a general decline, individuals born between 1947 and 1951 faced the highest disease risk, with relative risk(RR) values of 2.062(95%CI:1.703-2.496) for men and 1.848(95%CI:1.489-2.294) for women. However, disease burden projections still indicate a rising trend through 2030. Conclusion Since 2007,China has made significant progress in the prevention and control of occupational noise-induced hearing loss. However, young adults aged 20-<25 and 30-<40 remain high-risk groups requiring continued attention and protection. Future efforts should focus on collaborative prevention strategies across government, enterprises, and healthcare institutions to further reduce the burden of occupational noise-induced hearing loss.

With the rapid development of information technology and the advent of the era of mutual media, the boundary between work and life is increasingly blurred.Work connectivity behavior after hours, that is,dealing with work affairs through electronic devices after work or during rest, has become a common phenomenon in modern workplaces. Focusing on nurses as a special occupational group, this connectivity behavior can not only enhance work flexibility and emergency response efficiency, strengthen nurses' sense of professional efficacy and responsibility, but also become a core stressor that erodes nurses' physical and mental health, induces burnout, exacerbates work-family conflicts, triggers emotional exhaustion and various types of health problems, which threatens the well-being of the individual, the harmony of the family, and the stability of the quality of nursing care, showing an obvious double-edged sword effect. Therefore,it is particularly important to conduct in-depth research on work connectivity behavior after hours. This paper reviews the concept, theory, measurement tools, influencing factors, outcome variables and intervention strategies of work connectivity behavior after hours, aiming to draw the attention of managers and to provide references for the study of work connectivity behavior after hours among nurses. Through scientific management and effective intervention,it is expected to alleviate nurses' occupational stress and promote work-life integration, which is of great significance in safeguarding nurses' physical and mental health and optimizing the allocation of nursing resources.
