Objective To investigate the mediating role of nurses' general self-efficacy in their work environment perception and job burnout. Methods In March 2023,a convenience sampling method was used to conduct a questionnaire survey of 1 261 nurses from three grade-A tertiary hospitals in Baoding City using a self-compiled general information questionnaire, nursing work environment scale(C-NWE),Chinese version of Maslach burnout inventory(MBI),general self-efficacy scale(GSES),and the data were analyzed. Results The total scores of C-NWE,MBI,GESE for 1 261 nurses were (123.32±24.14),(65.80±23.33) and (28.27±6.83)points,respectively. The detection rates of zero, mild, moderate, and severe job burnout were 24.74%,24.66%,47.18% and 3.40%,respectively. General self-efficacy had a partial mediating effect between nursing work environment and job burnout, with an effect value of -0.097,accounting for 21.54% of the total effect. General self-efficacy had a moderating effect on the relationship between nursing work environment and job burnout(β=0.005,△R2=0.003,P<0.05). Conclusion The current situation of job burnout among nurses in three grade-A tertiary hospitals in Baoding is not optimistic and should be taken seriously by hospital managers. Nurses' general self-efficacy has both mediating and moderating effects between the nursing work environment and job burnout. Nursing managers should carry out targeted interventions from two aspects:the nursing work environment at the organization level and the general self-efficacy at the individual level.

Objective Noise is the main occupational hazard factor in automobile manufacturing plants. Through the uncertainty evaluation of noise measurement in welding workshop of a large automobile manufacturing plant in Tianjin, the source of uncertainty, quantitative calculation, evaluation method and reporting methods of measurement results were expounded. Methods According to GBZ/T 189.8-2007 "Measurement of Physical Factors in the Workplace Part 8:Noise",GBZ 2.2-2007 "Occupational Exposure Limits for Hazardous Factors in the Workplace Part 2:Physical Factors" and GB/T 21230-2014 "Engineering Method for Determination of Occupational Noise Exposure in Acoustics",from June 2022 to February 2023,the 30 representative operators from parts mounting position, assembly position and mold repair position in the welding workshop were selected for noise measurement. Using explosion-proof individual noise dosimeters, develop a reasonable measurement plan, read and record the A-weighted equivalent continuous sound pressure level(Lp,A,eqT,n) of three positions operators, calculate the A-weighted 8-hour exposure sound level(LAE,8 h) for each positions, and conduct uncertainty evaluation. Results Calculation showed that LAE,8 h of parts mounting position, assembly position and mold repair position were 83.8,84.6,80.5 dB(A),with expanded uncertainty of 3.3,3.6,3.3 dB(A)(k=1.65).The calculated LAE,8 h of parts mounting position and assembly position was below the exposure limit of 85 dB(A) but could include 85 dB(A)(confidence level:90%).The Lp,A,eqT,n of part of the workers in parts mounting position and assembly position didn’t meet the occupational exposure limit requirements for noise in GBZ 2.2-2007. Conclusion The evaluation of noise uncertainty characterizes the dispersion and degree of suspicion of noise measurement results, with the main source of uncertainty being measurement operations. It is recommended to maintain and update noise control facilities, strengthen on-site management of wearing ear protectors as required, and implement relevant health monitoring and management systems.

Objective To investigate the current status of the expectations and feelings of primary health services for patients with chronic diseases in Song County of Henan Province, and to explore the factors affecting the sense of acquisition of patients with chronic diseases through importance-performance analysis(IPA),so as to provide reference for improving the sense of acquisition of primary health services for patients with chronic diseases and perfecting the construction of the primary health care system. Methods From May to September 2023,a service quality(SERVQUAL) model was used to design a questionnaire on the expectations and feelings of primary health services for chronic disease patients. Using the IPA method,an IPA quadrant matrix and IPA index analysis model were constructed, and an actual survey was conducted on 671 chronic disease patients in Song County of Henan Province to evaluate their sense of acquisition in primary health services. Results The statistical mean of 24 factors was placed in 4 quadrants, and no one item index could reach a very high level of sense of acquisition. There were two indicators in the higher level of the sense of acquisition, which were located in the "over-performance" area. There were 13 indicators in the general level of the sense of acquisition,2 indicators were located in the "slow improvement" area,3 indicators were located in the "excessive performance" area, and 8 indicators were located in the "maintain advantage" area. There were 6 indicators in the lower level of the sense of acquisition,2 indicators were located in the "key focus" area, and 4 indicators were located in the "slow improvement" area. There were three indicators in the very low level of the sense of acquisition, two indicators were located in the "slow improvement" area, and one indicator was located in the "key focus" area. The total average score of service perception(1-5 points for "very low to very high") for 671 chronic disease patients in Song County of Henan Province was (3.74±0.63)points,and the overall service perception was at the upper-middle level. The scores of each dimension from high to low were empathy[(3.86±0.73)points],responsiveness[(3.8±0.7)points],reliability[(3.77±0.66)points],assurance[(3.66±0.71)points],and tangibility[(3.6±0.67)points]. The total average score of service expectation was (4.11±0.54)points,which was higher than the score of service perception. The total average score of service acquisition was (-0.36±0.6)points,but the perception did not reach the expected level. The smallest difference was in empathy[(-0.32±0.71)points],and the largest difference was in assurance[(-0.43±0.67)points]. Conclusion The internal environment of primary health service institutions is poor, the technical level of medical staff is relatively low,and the opinions and suggestions of patients are not taken seriously. There is a big gap with the expectations of patients, and the patients sense of acquisition is very low. The shortage of professional and technical personnel in primary health service institutions, the overall quality of medical staff is not high, the workload is large, which reduces the satisfaction and sense of acquisition of patients. Patients' expectations and satisfaction with medical examination equipment and basic drugs are low, which reduces patients' sense of acquisition.

Objective To analyze the status and health risk of groundwater pollution in rural area of a city in Northern Henan Province, provide scientific basis for improving rural ecological environment and promoting agricultural and rural modernization. Methods From January to December 2021,294 samples of private wells and domestic tap water in the study area were collected, and 39 indicators were tested. The correlation and sources among rural groundwater quality characteristics, pollution status and main pollution factors were analyzed by SPSS 24.0 software, and the health risk assessment was carried out based on the evaluation model of U.S. Enviromental Protection Agency(USEPA). Results 10 indexes of total hardness, soluble total solids, sulfate, chloride, manganese, oxygen consumption, sodium, fluoride, iodide and total number of colonies in the rural groundwater in the study area exceeded the Class Ⅲ standard limit of Groundwater Quality Standard(GB/T 14848-2017) to varying degrees. Eight indexes of total hardness, soluble total solids, sulfate, chloride, manganese, sodium, fluoride and total number of colonies exceeded the limit value specified in the Sanitary Standard for Drinking Water(GB 5749-2022). The other 29 indexes were better than or in line with the Class Ⅲ standard limit of the Groundwater Quality Standard(GB/T 14848-2017). The coefficients of variation of manganese, iodide and total colony were 2.00,1.25 and 2.16,respectively,which was mainly affected by human activities. The total hardness, soluble total solids, sulfate and chloride are homologous, which were mainly affected by regional stratigraphic factors, hydrogeological conditions and hydrogeochemical factors. The manganese was less harmful to the human health of adults and minors, and the amount of oral ingestion was within the acceptable range of human dose. The sample high value point of fluorine was more harmful to the human health of adults and minors, which exceeded the acceptable dose of oral ingestion, the chronic health risk of minors(HQmax=1.613) was 1.4 times that of adults(HQmax=1.145),while the human health risk of minors(Dmax=0.097) was 1.5 times that of adults(Dmax=0.069). Conclusion The fluorine should be taken as the key object of rural groundwater ecological environment control in this area,it is necessary to attract the attention of relevant departments, take relevant measures to strengthen the rural groundwater quality control, and improve the rural ecological environment.

Objective To analyze the performance of basic medical treatment and assistance funds for migrant workers with pneumoconiosis in Hunan Province, thus to provide a basis for government departments to continue to invest special assistance funds for migrant workers with pneumoconiosis in a reasonable manner. Methods The trend of changes in indicators such as the coverage and total number of basic medical treatment and assistance policies for migrant workers with pneumoconiosis in Hunan Province from 2017 to 2022,the proportion of special assistance for migrant workers with pneumoconiosis in medical expenses, the proportion of medical insurance reimbursement, hospitalization medical expenses, and the proportion of self-payment expenses. were analyzed. The utilization efficiency of basic medical special treatment and assistance funds for migrant workers with pneumoconiosis in Hunan Province was summarized, and the countermeasures for existing problems were analyzed and explored. Results Since 2017,the government in Hunan Province had invested 50 million special funds annually to implement medical treatment and assistance for migrant workers suffering from pneumoconiosis. It had been continuously implemented for six years until 2022. Through the three major measures of "Promotion,Reduction, and Assistance",the coverage and total number of patients who enjoyed the basic medical treatment and assistance policies for migrant workers with pneumoconiosis in Hunan Province continued to increase, the proportion of specialized assistance in hospitalization medical expenses were relatively stable, the proportion of patient medical insurance reimbursement were increasing year by year, the average medical expenses were decreasing year by year, and the proportion of patient out of pocket expenses were decreasing year by year. However, there were also three problems. Firstly, the incidence of pneumoconiosis among migrant workers in Hunan Province was still at its peak, the number of people receiving assistance had increased slightly year by year, and a death reporting mechanism had not yet been established. Secondly, the fairness of the use of rescue funds needed to be strengthened, with some patients repeatedly receiving assistance while others did not receive assistance throughout the year. Thirdly, certain limitations in the current policy of limited payment for individual medical insurance types for migrant workers with pneumoconiosis, led to the problem of the more urgent assistance needs of critically ill patients with high medical expenses could not be solved. Conclusion With the strong support of the government, the treatment and rescue work for migrant workers with pneumoconiosis in Hunan Province has achieved significant results, with good treatment outcomes and significant social benefits, achieving the goal of promoting people's health and maintaining social order and stability. However,it is still necessary to analyze and improve the problems that arise during the policy implementation process to maximize the benefits of limited rescue funds for the people.
